The Health Care Navigator role works with Veterans to provide case management and care coordination, health education, interdisciplinary collaboration, coordination, consultation, and administrative duties. This role will be responsible for a variety of direct service, technical and skilled tasks in accordance with the rules and regulations of the agency, funding sources and agency mission and philosophy. The Housing Resource Specialist will assess personal and family situations of individuals and families experiencing homelessness or at risk of homelessness to determine eligibility for programs and service needs. This role will be responsible for a variety of direct service, technical and skilled tasks in accordance with the rules and regulations of the agency, funding sources and agency mission and philosophy.
