Effective homeless response systems seek to reduce barriers to housing and provide voluntary support services for persons experiencing a housing crisis. Our programs and services are aligned around the common goal to get people into permanent housing and help them stay there.

We strive to ensure that anyone in need of housing resources can easily access our support. We work in collaboration with our community’s coordinated entry system to streamline the process of connecting individuals experiencing homelessness with our services.

Referrals through Coordinated Entry

Community Rebuilders receives all referrals through our community’s coordinated entry system. This approach allows our community to efficiently assess needs and match individuals with the most appropriate housing solutions and support services available.

Seeking Housing Resources

If you or someone you know is in need of housing resources, access support via:

  1. Online Assessment: Visit www.communityhousingconnect.org to fill out an assessment form. This step helps to understand your specific needs and circumstances better.
  2. Call United Way 211: Reach out to United Way 211, a vital resource for connecting individuals with community services. They will guide you through the process of accessing programs and services.

Permanent Supportive Housing

Permanent supportive housing is an intervention that combines affordable housing assistance with voluntary support services to address the needs of chronically homeless people. The services are designed to build independent living and tenancy skills and connect people with community-based health care, treatment and employment services. [1]

Rapid Re-Housing

Rapid re-housing provides short-term rental assistance and services. The goals are to help people obtain housing quickly, increase self- sufficiency, and stay housed. It is offered without preconditions (such as employment, income, absence of criminal record, or sobriety) and the resources and services provided are typically tailored to the needs of the person. Veterans’ Programs

We have a well coordinated and efficient community system that assures homelessness is rare, brief and non-recurring and no Veteran is forced to live on the street. This means that every Veteran has access to the supports they need and want to avoid staying on the street and move quickly to permanent housing.

Veteran homeless is now rare brief and non-recurring in Kent County.

Honoring those who’ve served, our specialized programs cater specifically to veterans experiencing homelessness, offering tailored services that address their unique needs and challenges.

Accessing Veteran Programs

While we encourage Veteran households to complete the Coordinated Entry Assessment as listed above, there is no wrong door for Veterans. In addition to the steps above, you may also call our office directly at 616-458-5102 to get information about Veteran Services.
