Community Housing Connect is an online gateway for Kent County families experiencing a housing crisis. The online platform is designed and managed by Community Rebuilders in coordination with the Kent County coordinated entry, HAP, operated by The Salvation Army. Community Housing Connect is an intuitive online assessment tool that breaks down barriers and builds up resources.
Once connected with Community Housing Connect, we are committed to resolving housing crises with the help of Solutions Specialists through Supported Solutions. Supported Solutions is the first point of contact, helping families to resolve their housing crisis through proactive problem-solving. The program is client-centered, flexible, and works toward immediate action.
Solutions Specialists are attentive listeners and solution-oriented thinkers, working with clients in creative ways to explore all resources and housing options available. The G.R.A.C.E. Network serves a vital role in connecting clients with all the resources necessary to resolve their housing crisis.
The G.R.A.C.E. Network provides a full circle of referral opportunities that are necessary to help stabilize a housing situation while also enhancing health & wellbeing.
