The G.R.A.C.E. Network has enhanced my work and service delivery in multiple ways. Utilizing the social determinants of health survey has become a vital resource that I use to serve our participants better.
The assessment brings attention to the struggles that would often be overlooked in the past. By recognizing the other struggles of our participants, I am able to make referrals for them to partner organizations that are able to assist them with the struggles they are experiencing. Being able to address these struggles has helped increase their overall wellbeing and has helped them to maintain their housing.
Another way that the G.R.A.C.E. Network has enhanced my work is by securely consolidating all the participant information in one place. Since the implementation of moving our file documents to G.R.A.C.E. Network, I have been able to access the information I need quicker and easier, especially while working from home during the COVID-19 pandemic.
